Is MP1584 really broken again? & sometimes the voltage is at 7.4V then drops to 5V from the SW MP1584 pin

First 5 pin chip L04A is LDO 3.3V
Second 5 pin chip 1202z is Step-Down DC-DC convertor

however the 5V voltage is still unstable.
are there certain components like resistors / capacitors & diodes which disturb the voltage of 5V from MP1584

1. I installed LP45 without 1202z. Then I connected the 5V line and only the buzzer turned on.
2. I installed 1202z without L04A, apparently the condition of 1202z was very hot.
it looks like these two components are the problem. or there are other ideas before making sure that the two components are the problem.

because of high voltage concern when using 3S batteries..

Maybe 1202z also hit but not like L04A with burning / burning condition.
is there another way to replace the 5V & 3V power.
Please help me.
